Chronic Disease Management
If you have a Medical Card or Over 70s Card or Doctor Visit Card
and have have any one of the following medical problems…
- Type 2 diabetes
- Breathing Problems {Asthma or COPD/Emphysema}
- Heart Problems {High Blood Pressure/ Irregular Heart Beat/ Atrial Fibrillation/ Previous Heart Attack/Stent}
- Stroke/Mini Stroke / TIA
Then you are eligible to register for our GP Chronic Disease Management Programmme.
Ask your doctor or nurse or inquire at at reception.
The Programme consists of one GP Nurse Consultation & one follow-on GP Consultation twice a year.
Depending on your illness a blood test will be taken free of charge once or twice a year.
The programme emphasises:
- Lifestyle advice
- Risk factor control
- Disease management
- Speak to your GP or practice nurse who can register you for this programme.
The HSE Chronic Disease Management Programme has been developed in line with international best practice and research.
This evidence shows that regular structured reviews of chronic medical conditions lead to improved patient outcomes through better condition management.
